About the role
Key responsibilities & impact- Deliver comprehensive virtual care, including patient assessments, condition management, medication review, and appropriate escalation, while ensuring compliance with telehealth regulations and licensure requirements.
- Collaborate with multidisciplinary teams and utilize telehealth workflows to support coordinated, continuous care and maintain accurate documentation of patient encounters.
- Educate patients on preventive care, disease management, and healthy lifestyle behaviors to improve outcomes and encourage active participation in their health.
- Provide compassionate, culturally sensitive care while maintaining professionalism, strong communication, and sound clinical judgment in all interactions.
- Adhere to clinical guidelines, regulatory standards, and organizational policies while participating in quality improvement initiatives.
- Ensure accurate, timely documentation, uphold patient privacy, and maintain high standards of safety, ethics, and confidentiality.
Requirements
What you’ll need- Active, unrestricted current NY licensure as a Provider in the state in which you will be providing care for patients.
- License must be clear of any clinical and/or board practice restrictions.
- NY State only: Minimum of 3600 hours of practice experience, or with 3 years of clinical experience (autonomous provider), in a primary care or longitudinal care setting
- Board Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
- Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements.
- Active Drug Enforcement Administration (DEA) license.
- Current Basic Life Support Certification for health care providers.
- Proficiency in electronic medical record (EMR) systems.
