About the role

Key responsibilities & impact
  • The RN Care Manager serves as a liaison among Brookdale teams, residents, families, and healthcare providers within our communities, promoting coordination, communication, and collaboration at Brookdale’s HealthPlus communities.
  • Works closely with primary and specialty care providers to formulate resident-specific care plans.
  • Serves as a resident advocate and contributes to improving service delivery.
  • Educates residents and families about care pathways.
  • Coordinates services to enhance wellness and manage chronic conditions.
  • Provides advanced care planning education and ensures completed Advanced Care Plans for residents.
  • Applies quality improvement methodology to analyze and manage outcomes.

Requirements

What you’ll need
  • Graduate from Accredited Nursing School and a minimum of 5-7 years of relevant experience required.
  • Knowledge of value-based care models is preferred.
  • Experience working in SNF and HH environments preferred.
  • Previous Care Management, Case Management, or Care Coordination preferred.
  • RN license required.
