About the role
Key responsibilities & impact- Conduct online “Video Visits” with health plan members
- Engage with health plan members primarily online, helping to close gaps in care
- Provide 40 hours of availability weekly
- Execute a strong visit completion rate
- Maintain compliance with operational, clinical, and member experience key performance indicators
Requirements
What you’ll need- Full-time, board-certified nurse practitioners or physician assistants
- Experience with Signify Health In-Home Health Evaluations, Video Visits, telehealth visit delivery
- Maintain training, affiliation, and compliance with Video Visit requirements
- Be trained and credentialed for all health plan clients in primary coverage state(s)
- Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)
- Able to obtain and maintain multi-state licenses as required
